Please fill in the form below for each attendee, except babies, and press the "Submit" button. There's no need to snail mail the form.

You can use your browser's "Back" button to base a new entry on the previous one. Yes, that means entering children as individual people, rather than as mere footnotes. "Babes in arms", however, are free. So they don't count as real people. Just mention their names in the "Other comments" for record-keeping. Thanks.

Payment

Please pay as soon as you register. There are two ways to pay:

Mail in a check payable to "Berkeley Morris". The address is given after you submit. Please don't fold it.
Paypal to mfj@ieee.org.
If you use Paypal, you need to figure out how to pay us the full amount without fees. Hint: use the "friends and family" button and pay from a bank account or Paypal balance. Send a check if you are not sure how to make this work!

Make sure it is obvious who you are paying for!
